There is a fee of $40 for a doctor’s note.
Notes will only be provided if you are treated at the Medical Centre on the day you are sick.
You are required to provide 24 hours’ notice to cancel an appointment. Please call the Medical Centre at 437-312-0370 and leave a message with your name, date, time of appointment and your phone number. Failure to notify the Medical Centre will result in a $50 missed appointment fee.
