To report changes to your OSAP application, you must:

  1. Download the OSAP Information Change Request form (PDF).
  2. Complete the form electronically using the boxes provided.
  3. Upload the form directly to your OSAP account. If you cannot upload it to your OSAP account, please submit the form to theservicehub@senecapolytechnic.ca using a valid Seneca email account.
